First check to see if you already have a DUNS number, if not obtain a DUNS number by continuing the steps after checking your entity:

a) Use the address and contact information in your corporate filing such as your articles of organization or most recent statement of information.

b) May require two legal documents with entity name, for example your Articles and a Statement of Information or Operating Agreement.
